The Farmington Valley Health District, in conjunction with the Farmington Valley Visiting Nurse Association, will provide COVID-19 vaccinations to any individual 18 years old and up who is unable to attend public clinics. The vaccine is Johnson and Johnson and is made available free of charge, with no billing to insurance or cost to the patient.

Residents must be from one of the following towns: Avon, Barkhamsted, Canton, Colebrook, East Granby, Farmington, Granby, Hartland, New Hartford or Simsbury.

The in-home vaccination focus is residents who are homebound, lack transportation or have other significant barriers to attend public clinics. Additionally, individuals who work as home health aides and have not received the vaccination may be eligible. Any family member providing care to a homebound person is also eligible. Any resident who has a history of severe allergic reactions like anaphylaxis or has required the use of epinephrine is not eligible for home vaccinations for safety reasons.

Please contact the Farmington Valley Visiting Nurse Association at 860-651-3539, Monday through Friday, 9 a.m.–330 p.m., to register for an in-home vaccination. The FVVNA will provide your information to the Health District, who will call you to confirm eligibility and schedule your appointment.
